Customers

New customers are required to complete these documents, available upon request:

  • Customer Credit Application

  • New Account Customer Profile (may be substituted by Purchase Order)

  • Sales and Use Tax Exemption Certificate (for domestic customers)

  • Declaration of Final Destination (for international customers or domestic with international shipping locations)

Vendors

  • Please request copy of Jayhawk new vendor form and submit W-9/W-8
